Reunion Registration

Registration for the 2011 Bushnell Reunion
Each shipmate attending the Bushnell Reunion "MUST"
print and completely fill out a Registration form and mail it to Mike
Wentzel along with a check for all monies due payable to the
"USS BUSHNELL REUNION".  This information is used for
planning purposes, giving us the number of attendees, dinner selections
and trip selections.  

If for some reason, you are unable to print a copy of the registration
form, E-Mail Mike Wentzel and he will mail you a copy.

PLEASE NOTE: This is NOT your Hotel Reservations.  Each
member is responsible for making their hotel reservations.

Registration Instructions

1. Click on the Sample Registration Form and make yourself familiar to what is on
the form and how we would like it filled out.  Print it out if you want.

2. Open the form for the Charlotte Registration.  Using the Sample Form as a
guide, type your responses to the Form.  You will be able to register up to 4
people on your first sheet.  If you need to add more, complete your first form
putting the total number who will be attending with you one this form.  Print the
form for mailing to Mike Wentzel.  Open a new form and add any additional
names plus their meal selections.  Be sure and let us know how many will be
attending the Hotel reception and Gedunk night.

3. Fill out all meal requests for those attending with you and enter the total dollar
value in the space provided.

4. Print out the form and send a check for the full amount to Mike Wentzel.  If you
have multiple forms, make sure to add those amounts also.  Multiple forms must
contain the Shipmates name in the space provided so that we can keep the
registrations together.
